Confirm Chang Leeas the provider and Q Functional Wellness & Chiropractic as the location.
Pick a date & time
Choose an available appointment time. Tuesday hours end at 12:30 PM; other weekday hours end at 5:30 PM.
Enter details and confirm
Book as a guest or sign in. Your portal account also lets you manage appointments, view documents, and check in.
